Regular Membership
STEP 1
Interested individual must open personally his/her Savings or Time Deposit Account. He/she must pay the required deposit and fees as approved by the Cooperative whether savings deposit or time deposit. The membership fee, ID Card, DPK contribution, and share capital passbook fee will be paid only upon approval of membership by the Board of Directors and when he/she deposits his/her share capital.
STEP 2
Fill up the Membership Application Form which can be downloaded below and Submit it with 2 pcs. 2×2 recent colored picture with your name and date, photocopy of any government-issued ID
STEP 3
The Membership Committee shall conduct a thorough investigation on the applicant and then recommend him/her to the Board of Directors during their regular meeting for the Pre-membership Education Seminar if qualified. If the applicant is not qualified, the application shall be denied.
STEP 4
Attend the Pre-Membership Education Seminar as scheduled by the Education Committee. The applicant shall pay the required share capital, membership fee, and other required fees within six months from the date of approval of his/her membership.
STEP 5
Payments and deposit:
Membership Fee: ₱ 200.00
Initial Share Capital Deposit: ₱ 5,000.00
Initial Savings: ₱ 500.00
Mortuary Fee (Voluntary): ₱ 5,000.00
DPK Program Contribution & Fee: ₱ 650.00
ATM Opening of Account Fee: ₱ 200.00
Total: ₱ 6,550.00
